When you have it downloaded, unzip it to %APPDATA%\.minecraft\saves or C:\Users\YourUserName\AppData\Roaming\.minecraft\saves

Start Minecraft and you should see all the worlds listed under Single Player.

If it hangs while loading, you may not have enough memory for Java 32-bit. To install Java 64-bit, get it from Java.com. Click on Windows Offline (64-bit), download it and install it.
